The path to rolling a new character is paved with good intentions. Namely, the intention that other players have to back you up, but for some reason, it never happened.
Share a time when your fellow gamers intended to help you out, but it just never quite happened.
If I had a nickel for every time I asked someone to bail my ranged character out of melee combat and they ignored me to go engage some other target, often as not ganging up with other melee specialists when I practically couldn't hit in melee without doubleteam bonuses, and I lost a limb...
That actually... didn't happen a lot with our PCs. With allied NPCs? All the time. I think it was one part the GMs not wanting to be "killer" GMs but also needing something that would motivate us. Worked reasonably well most of the time.

The other part was we were usually playing point-buy systems like GURPS, West End Games Star Wars, etc. plus we were picky players and GM's. That meant character creation was not speedy so (I think) none of us (different campaigns, different GMs) wanted to have to sit through character creations, then adjust the adventure for the new capabilities of the player in question.
Oh, but one time in an WEG Star Wars adventure, my character evaded an ambush that trapped the rest of the party. This apparently irritated the GM (I can't remember if my character did it in a "good" way or not), or maybe it was just natural that with the rest of the party in custody, the big bad dark Jedi was free to track me down personally.
That did not end well, as per the story time guidelines. ;)
