I did. That lich was observing the party for a while, and the fighter started EVERY fight by Improved Tripping his opponent and having a AoO-fest. Said lich used Fly before the fight, and faked that he was walking at all.
My party has a fighter who specializes in doing Ludicrous amounts of damage through rapid-fire archery. It's a good build, combining lots of attacks with lots of damage per attack. It'd be perfect except I realized that the build could be further improved by swapping out a bow for dual hand crossbows and the Two-weapon fighting tree. The resulting character gets 16 attacks per round and assuming sneak attack on all of them can deal an average of 700 damage per round. I had to make the Crossbows Merciful just to avoid making him a one halfling siege weapon.
He mentioned the Tree, so he probably has regular, Improved, and Greater Two-Weapon Fighting. With Rapid Shot and Haste you're looking at 8 attacks per round. And then assume this is 3.5 to get the Splitting enchantment which gives you two attacks for each arrow you fire and you get to 16 attacks.
You got it in one guess, that's right.
I should also mention he's wearing a Spare Hand belt to reload the crossbows for him, since that's the usual sticking point for ranged Dual-wielders.

That sounds like something out of the webcomic the Order of the Stick. I could totally see Xykon doing that.

He did, kind of. He had a flying mount, and when that was disabled he revealed he had a flying spell on himself.

You know, sometimes it's better to not stand up. I mean, what's a -4 to attack and effectively -4 ac when it means you can't be tripped again :P

What game is that? I know in Pathfinder that two-weapon fighting only nets you one extra attack with the off hand weapon.

Somehow I get the feeling that Luffy's quote of "I grapple the ship" is going to be turned against him...
