Health potions are actually a lot more expensive than medical bills. Adventurers only use them because there's no published permanent "shrink doctor" spell to allow you to take them with you.
...okay, I'm gonna hafta make a cleric with who uses Divine Metamagic and Persist Spell on Reduce Person to be smaller all day as a portable medic. Halfling, methinks, maybe with a Psychic Warrior dip or yet more wasted feats for further size shenanigans.
I'll probably never use it, but it'll be funny.

Your players now think he's perfectly fine. Maybe scratched, but fine.

They are out of health potions. The waiters jumped shipped because they ran out and pirates still come on a daily basis.

Isn't there a spell Reduce Person? I don't recall the duration, but with sufficient level, I think you could research a long lasting version.

Just add the Leadership feat, mix well, and it will come out as a fully portable doctor.
Or do it to an alchemist for a portable health potion dispenser. Insert spare cash (in proper reagent form), get potions on short order.

So, did Luffy actually roll for diplomacy this time, or did he use his 'special diplomacy dice' again?

Based on his reaction I think he actually did roll, whenever he used his special die he was calm about it.

My guess is that he wanted to roll normally, but automatically grabbed his "Diplomacy Die" and noticed just a moment too late.
