I've got a good one for this, okay, so, the DM for this campaign we were in came up with custom monster stats for Yamato-no-Orochi; and it was supposed to be fairly powerful but the party monk got a whole absurd chain of rolls and manage to basically lock it into a combo that took out all its health. Well, fast forward a couple months, the person who played that monk is DMing for a different group (at the same level) and needs a monster, so he swipes the snake stats for another battle. It turned out that the monster actually WAS exactly as tough as the first DM thought it was, and its only weakness was relatively low HP. With nobody in the second party rolling ludicrous damage; the endless cycle of poisoning striking and healing had the boss wear down the entire party into an eventual TPK.

One time, I was at a boardgaming convention (Kubla-Con) and I sat down to a demo of the GURPS system (4th E lite, I believe) with the GM and the only other player who showed up. We went against the monster the GM had cooked up, and we KICKED. IT'S. FURRY. @$$. Crits seven ways to Sunday, and with the hit location die GM was using, we had stabbed it in both hands and one of it's feet. The only good roll it got that encounter was to run away.

Afterwards, the GM informed us that this was beast meant to chew through parties of 6, and was to be the big bad for the game. He was flabbergasted we dunked it so easily with just two of us. Five minutes later and a mook zombie was wailing on us and almost killed the other player.

I encountered this DMing one of DnD 4ths published modules. There was a werewolf boss that recharged its ability to take an additional action during a turn when it rendered a pc unconscious or dead. It's hp and defenses where low but between regen and it's insane self recharging offense you basically had to blow it away in one go. Convention public parties are never that organized. My table barely ekked out there win but it left some them scarred for life. My significant other in particular now reacts to weres in games with the closest thing to a nuke she has.
