30 minutes in real life? Even in a normal game, that's only a few rounds of combat, at most half a minute of "real time"

Adding a real world timer is something I always thought would be a fun (and cruel) meta-power a GM could grant an enemy.
Other meta-powers include prohibiting players from communicating with each other, hiding the player's dice rolls from them, and confiscating player's notes.

For the record, all of which are optional, alternate, or default rules for many RPG systems.

There are also interesting things like in GURPS, where there are metagame traits like "Luck". Luck allows you to roll three times and select the desired result, and even allows you to use it after one roll has already been made (thus rolling twice more before selecting). Default version lets you use it once per hour of play, with better versions allowing once per 30 minutes and even once per 15 minutes! Timer resets when you use it.

You can however switch it to game time as well, as a modifier that doesn't change its end value. This is because in RPGs, players only have so much control of how time flows and it can just as easily backfire. Sure if you've got a large group combat might take long enough to get multiple uses of Luck in, and in a single adventure there might be many such chances indeed... but sometimes the adventure involves long travels and even though you so could use luck to help with rolls related to basic survival and such, an entire month's journey ends up happening in 15 minutes. ^^'

Drowning is really not a problem in combat, a decent con score of, say 16 gives you 32 rounds of combat before you even roll a check, if you stay calm... Which is slightly more than 3 minutes, reasonably realistic.

16, actually. Standard actions eat a round, at least in Pathfinder, I don't have the 3.5 books handy. Which is still a long time. But hey, I thought my barbarian cultists juiced up on enlarge person would have plenty of time to fight my pcs. They somehow managed to outlast them though, badly injured, but they made it through and after that the fight was a cakewalk for them.
