Once, when I was DMing, I had a fake villain hire a bunch of people to make an AI that took control of an entire planet. Then, when the PCs confronted him, the villain's sidekick (the real villain of that plot) killed him, took control of the AI and then told the party to leave. They asked what he planned to do, and he told them he planned to extort the planet and force them to all work for him using the AI. Surprisingly, they misinterpreted this, or just missed some part of it, and they actually walked away, letting him control the entire planet. No bluff check necessary.

I had to do a lot of damage control after that. And from then on, I learned I had to be blatantly obvious about when the villain was too far gone. Not much later, a PC delivered supplies to an arsonist, who then started pulling them out and setting them up all around the room. The building burned down within a few minutes, and the player didn't realize that they'd delivered arson supplies.

I have kept journals from my chaotic neutral character's perspective. They are fun to read to the group after the campaign so everyone has a good laugh, because they are laughably wrong.

Admittedly, he was drunk at the time. But at one point, my wizard walked into a tavern that had scorch marks all over the floor. People were screaming about monsters. And I, with my wisdom score of seven, shout over the crowd, "Has anyone seen my familiar? He's about yay high, made of fire?"

Even before this comic started, I'd always seen the scene that was coming up as a slice of "WTH?" Now, with the players, I can just see some of them saying, "Wait, hold up, I wanna see how many 1d4s she'll actually do to herself."

I routinely fall asleep during fights and tend to forget half of everything happening after the first session of a campaign. Not really a story, it's just how I roll.
