Coincidentally, almost the same thing happened in the game I gm just yesterday. The monk was challenged to a duel by an NPC samurai. an awesome fight broke out and the samurai was on 8 HP while the monk was on -1 at the end of it. 10 seconds later the paladin of tyranny in the party charged in yelling "my turn" and almost killed the samurai in a single attack.
So story time today is: Times players went off the rails only for the GM to have the rails erupt out of the ground like Cthulhu on a flaming Harley out of R'lyeh?
I have short one if anyone was interested.
My group was being gm'd by a new gm for a short campaign for him to get some practice. Apparently we were supposed to go to the local temple and check it out, but somehow due to shennanigans by us the players we missed all the quest hooks and the GM didn't want to be a "gm of the rings" gm, as a result he dropped weaker and weaker hints till he grew freustrated. Finally as we were leaving the city he finally said, "An earthquake shakes the city and the road before you tears asunder revealing a dank gray limestone cavern beneath. It is dark, and dusty. Roll for initiative."
And then we fought and chased a load of drow down into the underdark. Which considering we though the campaign was about fighting some kobolds nearby was an interesting turn of events, but as I said, we missed All the quest hooks.
It's basically impossible to miss the plot hooks in my games.
Mostly because I usually give the plot hook in the pitch. "The God of Cats (and Thieves) has just hired you band of classy rogues and ne'er-do-wells to steal the Tyrant's Regalia from the treasury atop the Lord's Tower", "The nation is in chaos after a devastating civil war - what a perfect opportunity for a group of enterprising young god-kings like yourself to take over" or even "You're a band of badass monster hunters who've just been hired to track down and kill a rampaging dragon". I make sure players know what sort of game it's going to be before they make their characters.
The times I don't do this I generally have the plot hook burst through the wall and try to murder them in the opening scene. Tends to grab their attention. ;p
