Oh! I got this one! Pretty sure I've told about this elsewhere, but I have a character who is a level one commoner(chef).

He has a feat that grants him barbarian-like temporary stat boosts once per day, for the purposes of obtaining ingredients. It adds something along the lines of 15 to jump, 20 to climb, a boost to swim, and boosts his speed and grants him the feat evasion temporarily. It is not a combat thing, though. It ends when he gets the item.

Let us suppose there is a super rare mushroom on a rock, in the middle of the edge of a waterfall. My chef would be empowered to leap into the water and grab the mushroom, and swim back with it.

Everyone though my character was some kind of useless joke. I used this feat to gather items like eagle eggs, mountain herbs, and at one point, I used the feat to snatch venomous eels out of the water, totally unharmed. Nobody thought anything of my character until we met a level inappropriate encounter. It was a T-Rex, andthe four of us were level three. There were some smaller things, probably bugbears, but the T-Rex was the big threat. Master chef Hannibal Mengele's eyes lit up like it was christmas, and he was a five year old Mother Theresa.

He proceeded to run just to the side of the T-rex, then run up a tree, and ninja flip onto its back, then carve its heart, lungs, and stomach out. The feat specifically stated that no roll of any kind was needed to gather the ingredients. I burned through three rages triple ensuring the beast was dead, and that we had enough for haggis.

I aced the rolls to get up there, due to my absurd temporary boost to jump and tumble.

The party feasted that night on haggis, stews, and T-rex tallow biscuits.

He even made gelatin out of the bones for dessert, flavored with fruit juices.

My character was capable of doing this from level one, but at level three, he had three of these boosts he could use, so I blew them all in one turn.

ah so THATS how this character worked, The "no roll needed to gather ingredients" clause I can't see how the DM did not see this incredibly obvious flaw. In fact if you were some race like a goblin who are willing to eat anything then there's absolutely nothing you can't instagib with that character.

Holy shit! And I thought Mage Armor, Animalistic Power, Haste, and Regenerate Moderate Wounds was good... Lion's Charge, too, and Sakkratar's Triple Strike on the standard action. But +22, to everyone, as a move? I am in awe.
War weavers are the best to cheese; because rather than overshadowing, their awesome boosts others'. ^_^
I guess I tend to favor turtling and winning by attrition when I approach a build. Which is stupid, considering how much more satisfying I find glass cannon victories. Granted, mixing Lion's Charge, Haste, and Triple Strike means not much should survive a good charge if I ever use that guy, so... generalizing VS optimizing, I guess. Both are deliciously cheesy.

Tried to respond three times, and it got lost each time, so I will make this brief. Hannibal Mengele was named after Hannibal Lecter, Hannibal from the A-Team, and Josef Mengele.

If anyone here knows who Josef Mengele was, then you would know not to underestimate this character.

He got a ton of skill points to make up for his low combat potential. Everything went into knowledge, cooking, foraging, survival, anything that could contribute to his cooking was used.

And nobody suspected anything until it was too late.

Well, you're not exactly wrong. I do love to surprise people with what my characters can do. I can be pretty decent at minmaxing, but I prefer to make amusing and interesting characters.

Sometimes, they are one and the same. Like my half halfling forest giant monk/ranger/whatever. His father was a proud, well respected halfling, and he was, in turn, accepted, despite his huge size. When I get a chance to play him, I will be taking the halfling rock throwing champion prestige class. With his size, he'll be able to throw a large boulder, hit his target, and have it bounce on top of them repeatedly, turning them from killed to finely ground.

I've tried, but none of my DM's or GM's would allow characters with abilities from obscure sourcebooks or web pages. Even spells in the players guide that were named after wizards from other campaign settings had to be researched first, and feats did not exist yet.

My best shot though was in Amber Diceless RPG, my "Referee" I beleive he was called allowed broken pattern initiates (but not broken or fixed logrus) and I tried to remove the flaw in it by walking the primal pattern in the way of the broken pattern.

My reasoning was, and I still beleive, that Zelazny had based the Pattern of Amber, on mystical labyrinths that are drawn on the floors of some castles. I had come to that conclusion long before the game was made back when I read the books.

This would mean that the lines could never cross or even come close enough to step from one line to another, giving plenty of room to walk between them! That would mean I could use my broken pattern abilities with none of the danger.

The Referee however argued that the Pattern was a celtic knot, and the crossing points were the veils, and even showed me some official artwork (Not drawn by Zelazny) that shows portions of the pattern with crossed lines and filigrees that would lead characters to a dead end.

The Referee sketched out the pattern for future reference, and later on I'm playing a blood of amber character trying to stop another from becoming attuned to the Jewel of Judgement. I tell the Referee that I'm going to cut ahead of the adversary to have an advantage at fighting him. He tells me theres no way to do that. I point to his drawing and say "I'll go that way where the lines cross and skip a couple of lines, I'm not leaving the pattern at any point, and I'm not missing anything since I'm already attuned to this pattern!"

"Snack Attack Crit"

Not much of a story but I was playing 5th ed and one of the wizards had a familiar who would use its help action to give my rogue advantage on his attacks turning his chance of critting to roughly 10% per attack instead of 5% per attack to say the least I got a lot of crits.The DM didn't think that sneak attack was effected by crit but a quick look at the rules showed that it was.I did a lot of damage.

I once made a wizard war weaver that could give the entire party +22 strength and +6 constitution with one action. There's this feat that gives all your transmutation spells that modify stats an extra +2, so I used bull's strength (+4), a combination of Gorillon's blessing (+2 arms and claws on all 4), fuse arms (You have two arms, +4 strength for each pair lost), and my war weaver was an arcane disciple of the courage domain, the 5th level spell being a unique transmuation spell that granted +4 strength and constitution, but as a morale bonus, so it stacked with the other spells (fuse arms was an untyped bonus). War weavers can store up to 4 spells in escrow and release them all as a move action, so that and a enlarge person (+2 strength +1 size category) makes 5 spells spread to the whole party (the war weaver's main schtick).

Yeah, he did not see the '4 different strength-boosting spells' bit coming.

While playtesting our superhero homebrew, the GM asked us to make combat oriented characters for a plot where Metas would have to fight team battles in the arena.

The first time she shouting "Oh crap" was when combat started: Another player and I had both decided to see how far we could abuse the initiate rules. Basically, you rolled a d10 and added your Initiative modifier (usually going from 0-10), if your ini went past 10, you got an additional action, same for 20, 30, etc. Using the Super Reflexes power both the other player and I were gathering +40-60 to our ini. I later did the math and realized they theoretically were moving at over 300mph.

To make it worse, we both had some synergies going that allowed us to increase both our evasion, damage reduction and our combat ratings, so we were rolling 14ish attack dice, and any attack with less than 15 dice couldn't deal damage at all.

Did I mention these were beginner characters?

I can't remember any stories of my own which have really broken a feat that I haven't already posted at some point or another, but I will offer some thirdhand ideas that I'm sure the readers here will appreciate if they don't already know them: The locate city bomb and the poultrymancer (note that the poultrymancer is especially funny because it is technically not a feat, but a flaw (or -1 feat) meaning that the average poultrymancer is stronger than the average commoner WITHOUT the chickens.

"Kobold Psion Insanity"

My kobold (who drives a warforged armor suit around for the hell of it and is rather crazy) found a psionic power that lets me first grab an enemy, then throw them around the battlefield into other enemies until they manage to break free. Or die. The DM was rather upset when his great-big battle against a small zombie horde was thwarted by a single spell, just grab the undead ogre and repeatedly throw him into the air and back into the weaker zombies.

I once helped my little brother make a Ninja/Fighter. Since I love the BoNS I got him to use two feats to get Child of shadows... This means he was concealed as long as he moved at least two spaces... so he got sudden strike... My DM who makes all sorts of OP as hell stuff decided that that combination was too powerful for the game. So we had to choose different feats. Ah well it was great while it lasted.

"The Bard"

I was running in a group that did not meet up often, due to lots of random stuff coming up last minute. I was playing a Bard in 3.5 with a bunch of silly fun feats. CG, in a party of questionable alignments. I took a flaw that my character hummed all the time and built a bard for melee combat, and back up in archery that never got used. Everyone initially joked that bards sucked. So I kept my bonuses to myself and buffed myself to fight. Well several session later (1yr in real time) We were gaming again and we got thrown into jail the GM completely forgot that I was a caster. The rest of the team escaped with the rouge and left me to rot. So in a court room with a bunch of evil wizards and low level fighters I killed them all with my defense speech. (it helped that it ended with perform oratory roll and greater sound burst please take 6d10 sonic damage to the room, the objects in the room, and the people in the room) A single casting of make whole later I had a functioning sword and killed the surviving guards on my way out.
The moral of the story never underestimate the bard.

Not solely feat and mechanics abuse, but definitely fits "they thought they knew the full scope":

My DM invented a prestige class in a previous campaign, and when the opportunity came up, she let my character take it.
The "shadowborn" class allowed teleportation through shadows, creation of shadow globes, and most importantly, the ability to produce and wield any weapon the character is proficient with. (magical bonus increases with level). My elven rogue basically stopped carrying other weapons, since he could produce anything he wanted as a +5 weapon with cold damage.

We reached level 16 or so, and she throws a demon in front of us. I'm second in initiative. During the first round, as the first player is finishing their turn, I'm looking for options on my character sheet.

approximate conversation:

Me: "I summon a bow, but I pull an arrow out of my quiver instead of creating one"
DM: "OK, you can do that. I didn't know you were still keeping any around." (This was the first non-shadow item I'd used in combat in almost a year, real-time.)
<roll>
DM: "yeah, you hit"
Me: "now it needs to make a will save"
DM: "OK..."
<crappy roll>
Me: "it's gone"
DM: [several seconds of shock] "what did you do?"
Me: "Do you remember back near the beginning of the campaign when you gave us those 3 arrows that send outsiders back to their native plane? I never got to use the last one. This is the right DC isn't it?"

Fortunately, that DM was extremely good at adjusting to change (She had to be. We derailed her plans at least once every session.)

I can understand the need for alibiing, even when I come up with dozens of different possibilities my players still catch me off guard. IE okay so you guys are going to try and diplomacy the giant monster.Rather than I don't know kill it or maybe run. them: nah. Just going to talk this out.

This happened after they slaughtered an entire village of orcs without a moments hesitation.

So, uh... Others are probably aware of this, and I know I'm a bit late to comment... But there's a maneuver from the Iron Heart discipline in Tome of Battle called Wall of Blades, which seems to be what that technique is based on, letting the user substitute an attack roll for AC. I have no idea which feat would let the user do something like that.
