There comes, every now and again, a moment. It is in this moment that you realize that you are directly responsible for the disaster about to unfold in front of you. This moment is mostly the same, not a new moment that is similar, but the same scene with slightly different elements. But, no matter the changes, this moment is always heralded by two words: "Oh crap."

The DM for the MLP games we're playing specializes in these kinds of things, the fact that our party consists of a bunch of morons doesn't "hurt" either.

Conversely, there are moments when a GM realizes that, due to a perfect storm of player stupidity and unfortunate circumstance, something incredibly, incredibly funny is about to happen.
I was running for a group that was trying to save some captive farmers from hideous flesh eating aliens. Having tracked the aliens, they found a cave. Hearing muffled voices from nearby, they decided that the best option was to simply drop the cave ceiling, crushing all within. One hit it with his gravity control, another used his elemental earth powers, and together, they slammed the rocks down with enough force to kill all of the farmers that were being kept in the cave.
The aliens, meanwhile, made their rolls to locate the source of this commotion, and so the several groups of aliens they were supposed to fight separately came at them all at once.
Really, it's the exact same sort of moment, just from the other side of the GM screen. Y'know, the side the laughter is coming from.

"The best plans"

I find that my best plans come from my players. I give them a situation and then let them try to figure things out. While they discuss, I listen in and try to discern what they think their worst-case scenario is. I had one player who would "figure things out" very quickly and see connections that I'm not nearly devious enough to consider during my prep work.

Me - "We have bounties on our heads, so we need to get to base unnoticed... *I look at he angel* ... Fuck."
NPC - "Well we can offer you this crate to conceal the Angel."
Angel - "Putting me in a Box? Please, I'll just fly real high and nobody will see me. Nothing will go wrong."
Gm - *roll* "Oh, look I rolled an hundred."
Everyone - *faceplam*

this one time in 40k dark heresy, i was playing a assassin sniper, some drughead ran up to me and entered melee, i had the great idea to just use a grenade, after all, i had a flack clock, and then the grenade got a critical, i lived, but had 1 hp for most of 2 sessions
