I like to take notes and keep a journal from my character's perspective. Then I read it back at the end of the campaign.

It's great for a good, long laugh.

"So there we were, surrounded by ghasts, when vampire humper and whore whisperer showed up, packing the big gun. We escaped, and told lord Kingly all about goatee's plan to steal his favorite kinging chair. We told him that we'd save his ass from discomfort, and we set off to catch goatee badman in the act."

Translation: So there we were, trapped by ghasts, when the paladin who will never live a certain incident down and the bard showed up with a ballista. He escaped, and traveled to the palace to warn the king that a necromancer threatened his kingdom, and aimed to take the throne for himself. We volunteered our services and set out to capture the foul villain.

This is an easy to follow one. You know how characters can be convinced that something else entirely is going on. It can get much worse.
