"Of course I'm prepared! I just need to review my notes..."

It happens to the best of DM's. As much as you might plan out a campaign or encounter, there's always the possibility of it not going as planned. Not really in the sense that the players decide to go off the rails and completely ignore the story, but in the sense that some situations might not take as long as they should, or something they had a really good chance pulling off went south.

It's in these cases that the DM's improv skills are put to the test, and they either need to speed up their process of creating the next area, or find a way to help the (willing) players get back onto the rails. Tell a story where the DM was caught completely off guard, intentionally or not.
